USC Name Badge Access Request Form

Student/Affiliate Badge orders require an account set-up by Southern California Trophy and approval by USC prior to authorization. Payment can be made via credit card or a university-authorized form of payment. To request an account, please read all the following directions and then follow them to create your account.

Step 1) Create SCTC Login (If you already have a SCTC login, proceed to step 2 below)

Use the screenshot below as a guide and follow this link which opens in a new window to create your new SCTC Login. After submission you will see an 'Account stored successfully' message. Close that window and return to this page to continue with step 2 below.

Step 2) Fill out the form below to request approval

Your information will be submitted to USC for account authorization. If your account is authorized, you will receive an email from Southern California Trophy notifying you that the username you provided in your request is now authorized to purchase USC Student/Affiliate Name Badges.
